Health Forms (asthma, epi-pen, medication, & school physicalS) are located under health/wellness forms
It is a New Jersey state law that all medications must be brought in by a parent or guardian. Please contact me if you
need to make special arrangements before or after school.
A change of clothes is a good idea if you work during the day or are far from home. Spills/accidents happen!
If you have a phone number change change, please contact me. It is important to have current numbers in case of emergencies.
If your child has a fever above 100 degrees, please have them stay home and rest for 24 hours before returning to school.
If your child has vomitted, they must be vomit free for 24 hours in order to return to school.
Please check with your child's teacher and class mother about food allergies in your child's classroom, especially peanut allergies.
All of these healthful hints are to ensure your child as well as the other children at the Donahue School, remain healthy and well.
Thanks for your cooperation.
